Our pharmacists offer patient counseling on proper medication use, including dosage, administration, potential side effects, and interactions with other medications.

Chronic Disease Management

We assist in managing chronic diseases such as diabetes, hypertension, and asthma by providing education, monitoring, and support to help patients manage their conditions effectively.

Medication Management

Our pharmacists assess and manage medication therapy, conduct medication reviews, identify drug therapy problems, and provide recommendations to optimize therapy

Vaccinations and Immunizations

Our pharmacists are authorized to administer a variety of vaccinations, including flu shots, travel vaccines, and other routine immunizations recommended by public health authorities.

Patient Assessment and Screening

Before administering an injection, pharmacists conduct a brief assessment of the patient's medical history, current health status, and any contraindications or precautions.

Documentation and Reporting

We document the administration of injections, including details such as the type of vaccine, dosage, lot number, and injection site, and may report immunization data to public health authorities.

Smoking Cessation Program

Many pharmacies offer smoking cessation programs, counseling, and nicotine replacement therapy. Inquire at your local pharmacy about available options.

Cardiovascular Program

This program is available to patients in Ontario who have both high blood pressure and high cholesterol, have extended health coverage with Green Shield Canada, and are under 65 years of age.

Medication Reconciliation is a way to make sure that your most up-to- date medication list is available to all members of the health care team looking after you. This helps us make sure you take the correct medication

All residents of Ontario who take 3 or more prescription medications are eligible.

We need your help for Medication Reconciliation to be successful. You play an active role in your care by helping health professionals gather your medication list.

MedsCheck is a free 30 minute conversation with your community pharmacist where your medications are explained. Your pharmacist then gives you a form that you bring to your Doctor appointment. This helps your doctor keep track of all the medications you take and prevents any mistakes.

You can prepare for your MedsCheck appointment by bringing the following items to your scheduled meeting:
• Your OHIP card
• Any current medication containers, including those from other pharmacies or a list of all your medications
• Any non-prescription medications, vitamins, and/or herbal products you currently take
