Your medications are always changing. As new medications come on the market or your health conditions change, your doctor may make changes to the medications you take on a regular basis. Often, these changes can be overwhelming and gaps in care may occur.
That’s where the MedsCheck program can help! A MedsCheck is a free face-to-face appointment with one of our trained pharmacists, with the goal of educating you on your medications, their side effects and to find, discuss and fix any medication errors that may have been missed over time
Who Is Eligible?
Anyone with a valid Ontario Health Card who meets one of the following criteria:
(1) Taking 3 or more chronic prescription medications
(2) Anyone diagnosed with type 1 or type 2 diabetes
What Does It Entail?
A MedsCheck is a free annual program that allows you to sit down with one of our trained pharmacists in a face-to-face encounter. The time it takes to do a MedsCheck varies and depends on how many medications you are on, how many questions you may have and if any issues are identified. The goal is to give you a non-rushed encounter with one of our pharmacists to help educate (or re-educate) you on your medications. We want you to know what medications you are on, what they are for, ensure you are not experiencing any adverse effects, help you know what to watch out for and more!
Why Should I Get A MedsCheck?
Medications are complicated, they interact with each other in many ways. Quite often you’ll be prescribed a medication for one condition, experience a side effect of it and unintentionally be prescribed another medication for that side effect, this is known as the prescribing cascade. This is one of many problems we look for when doing a MedsCheck. The goal of a MedsCheck is to ensure you are being medicated safely and to the best of our ability. We will use what we learn from the MedsCheck to coordinate any changes we may feel prudent with your doctor directly. Your health is important, and our goal is to make sure we keep you as healthy as possible!
How Often Can I Get A MedsCheck?
We recommend our patients get a MedsCheck once a year. However, your health and medications may change sooner than that. We can schedule a follow up MedsCheck with you earlier than that if deemed necessary, talk to our pharmacy staff today!
What Does It Cost?
Nothing! MedsChecks are covered under the Ontario government, similar to how your doctor’s appointment is covered. As long as you meet the eligibility criteria, and haven’t had a MedsCheck in the past year, you can schedule one with us today!
